Massage Therapy Terms & Conditions Agreement *

By booking an appointment with Sacred Mama Denver, LLC, you acknowledge and agree to the following terms and conditions: 1. Appointment Confirmation Clients are responsible for confirming their appointments. If you need to cancel or reschedule, please provide adequate notice as outlined below. 2. Late Arrivals Late Arrival Policy: Please arrive 10 minutes before your scheduled appointment to complete any necessary paperwork and prepare for your session. If you arrive late, your session may be shortened to accommodate other scheduled appointments. You will still be charged the full rate for the appointment. If you are more than 15 minutes late, the therapist reserves the right to cancel or reschedule your appointment and charge you the full session fee. 3. Cancellations Cancellation Notice: We require at least 24 hours' notice for cancellations or rescheduling. This allows us to offer the time to another client on the waiting list. Late Cancellations: Cancellations made within 24 hours of your appointment will result in a charge of 50% of the total session price. No Show: If you fail to show up for your appointment without prior notice, you will be charged the full price of the session. 4. No Show Policy If you do not show up for your scheduled appointment and do not notify us in advance, you will be considered a “no-show” and will be charged the full session fee. Repeated no-shows may require prepayment for future appointments. 5. Prepayment for Services In some cases (e.g., frequent late cancellations or no-shows), prepayment for services may be required at the time of booking. 6. Rescheduling If you need to reschedule, please give us as much notice as possible to allow time to accommodate other clients. Rescheduling within 24 hours of the appointment will be subject to the same 50% fee as cancellations. 7. Emergency Situations We understand that emergencies happen. If you must cancel or reschedule due to an emergency, please contact us as soon as possible. We will do our best to accommodate you and may waive cancellation fees on a case-by-case basis. 8. Special Considerations If you are feeling unwell or suspect you may be contagious, please reschedule your appointment to avoid spreading illness. We will not charge you for cancellations made in this instance if you notify us as soon as possible. 9. Fees & Payment All fees for missed or canceled appointments will be charged to the card on file or billed directly to the client. Payments for services are due at the time of the appointment, and we accept [list of accepted payment methods]. 10. Client Responsibility It is your responsibility to arrive on time and cancel/reschedule your appointment with enough notice to avoid penalties. We reserve the right to refuse service to clients who do not respect the cancellation policy or repeatedly fail to honor scheduled appointments. 11.
